Extinction Therapy Fails to Engage With Impaired Brain Region in Chronic Cocaine Users

Individuals who chronically abuse cocaine gradually find that the substance is less enjoyable and its rewarding effects significantly diminished after years of use. Despite the reduction in rewarding effects, individuals often experience a hard time quitting cocaine. This fact also holds water in the case of most addictive substances that reflect similar changes in the derived pleasure due to chronic abuse.

An addiction characterized by persistent drug-seeking behavior is partly driven by a learning process that plays a key role in internalizing the cues associated with drug use. Extinction learning, a psychotherapy process which which conditioned responses to neutral cues (conditioned stimuli) gain effective properties when paired with an arousing event (unconditioned stimuli), is often used in treating anxiety disorders, such as phobias and post-traumatic stress disorder (PTSD) ).

The potential of extinction learning in replacing the learned responses to drug cues with healthier responses has been publicly debated as an option to treat individuals with the problem of addiction. However, a new brain imaging study published in the journal Addiction Biology argues that extinction-based therapy may not be as effective for cocaine users.

Neurobiological impairments diminish effectiveness of extinction therapy

The study conducted by the researchers at the Icahn School of Medicine at Mount Sinai discovered that the long-term cocaine use was associated with a “global impairment” in the ventromedial prefrontal cortex (VMPFC), a brain region associated with controlling impulsivity and maintaining self -control. VMPFC along with the striatum combines the brain's “valuation system” implicated in eliciting cravings by internalizing learned and reinforced drug-associated cues.

Based on the model of classical fear-conditioning studies, the researchers examined the correlation between the VMPFC and extinction learning in the case of chronic cocaine users by examining the 18 nontreatment-seeking participants with cocaine addiction and 15 healthy individuals. The participants were engaged in a two-day functional magnetic resonance imaging (fMRI) study that entailed three-phase classical conditioning paradigms:

  • Stage 1: In this acquisition phase, participants learned to associate an abstract cue with drug-related or pleasant images.
  • Stage 2: The two groups were involved in the extension learning where abstract cues were repeatedly presented without the corresponding images.
  • Stage 3: On the second day of the study, the groups were assessed by their ability to retain their extension learning from day one.

According to lead researcher Anna Konova, Ph.D., “Our study data suggest that it will be hard for long-time cocaine users to unlearn what once was a positive experience if this 'unlearning' or new learning relies on this brain region to be effective. ”

The other key finds of the study are as follows:

  • It was observed that long-term cocaine users had trouble forming and maintaining new associations for stimuli that were previously (although no longer) predictive of both drug-related and nondrug-related outcomes. This injury was mediated by the VMPFC.
  • The VMPFC signals of the cocaine-using group did not resemble those of the control group. Since extinction learning did not affect the VMPFC to the same degree, it suggests the failure of extinction learning in addressing the challenges of the cocaine-using group.

The researchers explain that extinction-based therapy, when applied in isolation or in its traditional form, is likely to be ineffective in addressing cocaine addiction. When the VMPFC is compromised due to the chronic cocaine use, it leads to a neural circuitry that does not support such learning. In addition, since these shortcomings are seen in both drug-related and nondrug-related scenarios, the researchers allude that extinction therapy may not work in most cases.

Since cocaine addiction is a serious public health threat in the United States, there are not many effective treatment approaches for this disorder. The function of the prefrontal cortex can be divided into a “go” system that controls impulsive behavior and habitual responding and a “no-go” system that inhibits such responses and exercises control over the “go” system. Research has detected that heightened activities in the “go” system or low activity in the “no-go” system can contribute to binge drinking by regulating the activation of the brain stress systems and inhibiting the response to alcohol-related cues. Furthermore, these mechanisms also control how a person experiences cravings and succumbs to a relapse.

The three-stage model of AUD not only explains how alcohol can take over the brain, but also explains the impact of quitting its habit. The changes in the neurotransmitters and other activities of the brain following alcohol cessation persist for long, even years before the brain naturally stabilizes. Recovery is not a simple task, especially when addiction is so multifaceted in nature in AUD patients. Fortunately, the disorder is treatable in the right medical set-up and individuals diagnosed with it can attain sobriety.

How to Protect Our Bodies From Endocrine Or Hormonal Disruption

Most of us tend to ignore the reports in the news every other day about how chemicals, pesticides, electro-magnetic fields and radiation can mess up our systems. Many of these have become part of our lifestyles – like the morning after pill, birth control pills in general, house dust that might have lead in it, in-organic vegetables and fruits and worse – lack of exposure to sunlight.

When we do this, we allow our endocrine system access to the source of its balance, sunlight. If we are sleeping during the day, or are not getting enough exposure to sunlight (it needs not to be hard direct sunlight) our cells are not able to absorb the Vitamin D that a very important endocrine gland in the Brain – the Pineal gland – needs to do its work. When the Pineal is not healthy, the nervous system is disrupted due to mental and psychological problems, sleep disorders and these go on to cause serious hormonal imbalances which lead to faster aging, skin and bone problems, and reproductive system imbalances.

2) Watch where you get your food from.
